By signing this document, you agree to abide by the TimberstoneVet policies as follows:

Payment is due at the time of service. Current accepted forms of payment include; cash, check, Mastercard, Visa and American Express. A fee of $25.00 will be applied for all returned checks, and when applicable, associated legal fees will be considered payable. A 1.5% per month service charge will be applied to all unpaid accounts. Account balances not collected within 60 days are subject to collection fees, including associated legal fees.

A pet is considered abandoned if, within 5 days of written notice to do so, the pet is not picked up from TimberstoneVet by an authorized person. If abandoned, the clinic may rehome, or if medically necessary, euthanize the abandoned pet. Services provided to the pet will remain payable by the client until the pet is transferred out of TimberstoneVet’s care.

Pharmaceuticals may not be returned for a refund once they have left the building.

TimberstoneVet reserves the right to take photographs of my pet(s) for the purpose of advertising and teaching/education. Pets may be identified in coordination with their photo on a first name basis.

We are proud smoke-free environment. Please refrain from smoking on TimberstoneVet property.

Late appointment policy: We ask that you arrive 5 minutes before your appointment time. Appointments that are more than 5 minutes late will be asked to reschedule. This policy ensures your pet receives the necessary time with the doctor. Repeated late arrivals may be asked for a nonrefundable exam fee deposit. If the appointment is on time and kept, the exam fee deposit credit will be applied to the invoice.
